pädiatrischen Alter
• f/m-Ratio 4-7:1
• genetische Veranlagung (im weitesten Sinne für Autoimmunität)
• Umwelt: Iod-Exzess, Infekte, Trauma, Rx-Therapie
• Anti-TPO sowie Anti-Tg-Ak als Marker der Zellschädigung (nicht kausal
– Folge!), 10-15% negativ!
• TSH-Rezeptor-Ak: i.d.Regel stimulierend (Basedow), aber auch
inhibierend! Können den Effekt im Verlauf ändern!
• Prävalenz nicht so klar, auf jeden Fall zunehmend
• Diagnose: Labor + US-Bild (hypo-echogen, heterogen, aufgelockert,
Perfusion ++)
• Assoziation rel. oft mit Vitiligo, Alopecia, T1DM, Zöliakie, selten i.R. APS

• 75%: Rezidiv in den ersten 6 Monaten nach Stopp

• langer erster Th-Block scheint die Rezidivrate zu
senken
• CAVE: TRAK lebenslänglich, auch nach

Take Home Messages/I• Wachstumsgeschwindigkeit und Perzentilkurven führen
– es lohnt sich!
• eine wiederholt normale WG (zwischen P10 und P90) schliesst eine hormonelle Störung zu 99% aus!
• Next step bei Unklarheit: Rx Hand links
• Das Bild sollte sich immer im Rahmen befinden! Gross-und Kleinwuchs-Syndrome selten, aber möglich
• Ein Mädchen mit einer Prognose unterhalb des EZB gehört immer auch mit Karyotyp abgeklärt (XO)!
• Ein Knabe mit Stimmbruch und HV 4 bis 8 ml ebenfalls (XXY)

• Auffälligkeiten bei SD-Kontrollen, insb. erhöhtes TSH, sind häufig. Lockere Nachkontrolle genügt meistens, falls Auto-Ak negativ
• Bei zu tiefer Wachstumsgeschwindigkeit im Alter 4 bis 10-12: Bestimmung von TSH, fT3, fT4, IgA tot, TTG-IgA, IGF-1, IGF-BP3, Leber und Nierenwerte, BB, E’lyte
• Zusätzlich über 10-12j: LH, FSH, Testo (Buebe), Estradiol (Maitlis). Evtl. BSR, CRP (IBD?)
• immer auch ein Auge auf Gewichtsverlauf: Kaloriendepletion, Kalorienmangel?
• «Früh-normal, Spät-Normal, normal klein, normal gross» sind immer noch die allerhäufigste Diagnose…
