Schwerwiegend: Schock; DIC; akutes Nierenversagen
Ansonsten: Fieber, Schüttelfrost, Unruhe, Angst, Flush, Übelkeit, Erbrechen, Tachykardie, Brust- Rücken- Flankenschmerz, Dyspnoe, Hypotonie,, Blutungen, Oligurie, Hämoglobinurie;

akute HTR
Ursachen ABO-major-inkompatible EK:
Fast immer Verwechslung („clerical error“) Präformierte Allo-AK (z.B. Vortransfundierte, Schsch) AB0-minor-inkompatible plasma-haltige Präparate
Frequenz Verwechslung: 1:6'000 bis 1:33'000 Tödlich: 1:100'000 bis 1:500'000

Überprüfung der Patienten- und Produkte-Identität
Klärung, ob eine „Überkreuzverwechslung“ vorliegt!
Mit post- und möglichst auch prätransfusioneller Blutprobe
des Patienten: Blutgruppenbestimmung, AK-Suchtest,
Kreuzprobe, direkter Antihumanglobulintest (DAT)(= „direkter Coombs-Test“); Kontrolle der Blutgruppe des EK
Hämolysediagnostik (Blutbild, LDH, Haptoglobin...)
Umgehend Protokollierung des Ereignisses durch beteiligte
und verantwortliche Fachperson(-en)
Meldung an Swissmedic (gesetzlich vorgeschrieben!)
Vorgehen bei V.a. akute HTR
20.11.2011, Behrouz Mansouri Taleghani 9
Universitätsklinik für Hämatologie und Hämatologisches Zentrallabor
Verzögerte hämolytische Transfusionsreaktion
Immunisierung durch Schsch oder Vortransfusion
Ursache: Blutgruppenantikörper, deren Titer vor der
Transfusion unter der Nachweisgrenze liegt
Boosterung der Antikörper (selten rasche Primärantwort!)
Hämolyse > 24h bis 28 Tage nach Transfusion
Symptome weniger schwer als bei akuter HTR
(„serologische verzögerte HTR“ = in vitro = ohne nachweisbare
Hämolyse)

Transfusionsassoziierte Volumenüberladung (TACO) Cave: ältere Patienten mit Herzinsuffizienz, Neugeborene, Säuglinge
Mindestens 4 der 5 Symptome innerhalb 6h
- akute Atemnot
- Tachykardie
- Blutdruckanstieg
- Akutes oder zunehmendes Lungenödem
- Positive Volumenbilanz
- (eine Erhöhung des BNP-Serumspigels unterstützt die Diagnose)
Prophylaxe: langsame Transfusion über z.B. 4 – 6 h, allenfalls Splitting
der Produkte und zweizeitige Gabe

TRALI: Preventive Measures Adherence to guidelines of blood utilization, especially FFP/PLT
Identify implicated donor and exclude them from future donations
(Problem: what about donors without antibodies?)
Cave-at in case of co-component donations
Exclude of multigravida/-parous from plasma containing blood donations
Alternatively: introduction of antibody screening tests in those women?
Pooling of plasma (e.g. S-D-FFP)?
Non immune TRALI: Use of fresh and/or washed blood components in at
risk patient groups (haematological diseases, cardial diseases)?*

Transfusion Volume 40 Issue 11 Page 1285 - November 2000
The blood bank factory, circa 2005. A unit of RBCs prepared from the donor (group A, D+ in this example) is processed by the blood bank factory, with successive machines performing WBC reduction, pathogen inactivation, enzymatic conversion (removal) of the A antigen, and pegylation to mask all non-ABO antigens, including D. The final product, a WBC-reduced, pathogen-free, stealth ECO RBC, is a universal RBC for transfusion to any patient, regardless of ABO group, D phenotype, or the presence of alloantibodies or autoantibodies to any RBC antigens.
